Сирингомиелия – хроническое прогрессирующее заболевание нервной системы, при котором в спинном мозге образуются полости. Особый интерес представляет идиопатический тип сирингомиелии, который ассоциирован с возникновением полостей в спинном мозге без явной причины. В данной статье отражено описание клинического случая сирингомиелии, верификация причины которой вызвала значительные сложности и которая, как следствие, могла быть отнесена к идиопатическому типу.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. Представить на обсуждение сложный с точки зрения диагностики случай сирингомиелии, а также предложить диагностическую тактику при затруднениях в верификации ее причины.</p><p>В работе представлен клинический случай пациента, обратившегося в ОГБУЗ «Иркутская городская клиническая больница № 3» в январе 2023 г. с жалобами на выраженную прогрессирующую слабость в ногах (больше слева), умеренную слабость в руках, снижение чувствительности в нижней половине тела (больше справа), нарушение температурной чувствительности на спине, животе, учащение мочеиспускания, слабость в левой руке с декабря 2022 г. В течение нескольких лет обращений за медицинской помощью, в ходе которых неоднократно проводились магнитно-резонансная томография (МРТ) и МРТ с внутривенным контрастированием, причину заболевания выявить не удалось. Было проведено комплексное обследование, включая контрастную мультиспиральную компьютерную томографию с миелографией, по результатам которой был выявлен блок распространения контрастного вещества в заднем позвоночном субарахноидальном пространстве. Было проведено хирургическое лечение в объеме выполнения ламниэктомии ThIII–ThVI и микрохирургического удаления объемного новообразования позвоночного канала на уровне ThIII–ThV с использованием интраоперационной ультразвуковой навигации. Послеоперационный период протекал без осложнений, был отмечен регресс неврологической симптоматики. Период наблюдения составил более 3 месяцев с благоприятным исходом.</p><p>По данным гистологического исследования было установлено, что причиной сирингомиелии у пациента А. являлась менингиома менинготелиального типа G1, которая не накапливала контрастное вещество.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Syringomyelia is a chronic progressive disease of the nervous system characterized by the formation of cavities within the spinal cord. Idiopathic syringomyelia, which is associated with the development of spinal cord cavities without an apparent cause, is of particular interest. This article describes a clinical case of syringomyelia where determining the etiology proved to be significantly challenging, to the extent that it could have been classified as idiopathic.</p></sec><sec><title>Aim of the study</title><p>Aim of the study. To present a diagnostically complex case of syringomyelia for discussion and to propose a diagnostic approach for situations where verifying the cause is difficult.</p></sec><sec><title>Case description</title><p>Case description. The paper presents a clinical case of a patient who was admitted to the Irkutsk City Clinical Hospital No. 3 in January 2023. The patient’s complaints included severe progressive weakness in the legs (more pronounced on the left), moderate weakness in the arms, decreased sensation in the lower half of the body (more pronounced on the right), impaired temperature sensitivity on the back and abdomen, increased urinary frequency, and weakness in the left arm since December 2022. Over several years of seeking medical help, magnetic resonance imaging (MRI) and contrast-enhanced MRI had been performed repeatedly but failed to identify the cause of the disease. A comprehensive examination was conducted, including contrast-enhanced multispiral computed tomography with myelography, which revealed a blockage of contrast agent propagation in the posterior spinal subarachnoid space. Surgical treatment was performed, involving laminectomy of ThIII–ThVI and microsurgical removal of a space-occupying lesion in the spinal canal at the ThIII–ThV level using intraoperative ultrasound navigation. The postoperative course was uneventful, with regression of neurological symptoms noted. The follow-up period was over 3 months with a favorable outcome.
